What do the Mental Health Outreach Team Do?

The Outreach team aims to assist people to withdraw from short and long term mental health services whilst preventing others from coming into mainstream services.

We do this in a variety of ways.

The outreach star support are short term individually tailored packages of support lasting for up to 10 weeks duration. Individuals self assess using the mental wellbeing star and from this assessment we mutually agree two or three achievable practical goals that if achieved, will benefit the individual and help to lift their mood. At the end of our period of support, the individual will reassess themselves so they can map their progression. Upon closure we always ensure that individuals have been referred on to or signposted to other services and activities.

The Outreach team takes referrals from the following agencies:

  • North West Boroughs Assessment and home treatment teams
  • NWB Recovery team
  • NWB Inpatient wards
  • Early intervention in psychosis team
  • Independent recovery and treatment centre
  • WGH Hospital discharge team
  • WBC First response team
  • WBC Neighbourhood complex streams
  • CGL Pathways

Outreach Group Therapies

The outreach group provide a range of group therapies in topics such as Positive thoughts, Anxiety management, Emotional freedom technique, Life skills therapy and Tai Chi. All of these groups teach the individual, practical skills which they can use to help aid relaxation and manage their mental wellbeing.

Creative Remedies

Creative remedies include a range of arts and wellbeing activities that offer occupation and the chance to learn new skills in a friendly supportive environment. Individuals can build their confidence and meet new people and make new friends. The aim of creative remedies is to assist people to develop skills that will assist them to return to further training, education or employment.
